The 12-step RCA process is a powerful tool for solving complex and recurring problems. It allows us to drill down to root causes, establish causal relationships, and generate creative solutions that solve problems continuously. More robust tools like the 12-step RCA are typically used for high-risk problems. Ideally, RCAs are multi-departmental activities, so the involvement of stakeholders affected by the problem is essential to ensure that all perspectives are considered.

Workshop Methodology
This course is presented in a group setting with the participants involved in team-based activities. Trainees work through various scenarios around events, conditions, and combination causes. Work is in small groups RCA’s on real problems from the trainee’s workplace. Attendees are guided through the 12-step process, including discovering innovative solutions to their real problems, which culminate in a business case with recommendations.
